First Visit

The main goal of your first visit is for us to get acquainted and learn as much as we can about your health (both dental and medical), any current concerns or issues that you have and your lifestyle. A variety of risk factors can affect your dental health, including stress, teeth grinding/clenching, genetics, smoking, diabetes and compliance to preventative care .

PROMPT EMERGENCY DENTAL CARE

With five dental specialists at the South Florida Center for Periodontics & Implant Dentistry, there’s always someone on call for emergencies. Patients who call after hours can speak to a periodontist by phone. During regular office hours, we make time in our schedules for patients with urgent needs. Our goal is to see emergency cases promptly, alleviate symptoms to make you comfortable and provide necessary treatment.

New Patient Forms

Please fill in these new patient fillable forms and return to us at mailbox@flsmile.com prior to your first appointment:

We make every effort to stay on schedule to minimize waiting at the time of your appointment. Sometimes emergency patients can cause delays to the schedule, and your understanding and patience are very much appreciated.

Your First Visit:  What to Bring With You

You may be surprised at how much time is spent with you during your first visit, performing a thorough periodontal and oral cancer examination, taking necessary x-rays and discussing your medical and dental history.

Please assist us at the time of your initial visit to the office by also bringing the following information:

  • A referral slip and X-rays from your referring dentist (if available)
  • A list of medications you are currently taking
  • Any dental insurance claim forms or cards
  • A photo ID

Please Note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, diabetes, or hypertension) or are on any blood thinning medicines including aspirin therapy.
